The Truth About Marijuana and Glaucoma

Glaucoma is a progressive eye condition characterized by damage to the optic nerve, often related to elevated intraocular pressure (IOP). Marijuana, derived from cannabis, has been explored as a potential means to lower that pressure. Researchers have studied cannabinoids for their IOP-lowering properties and possible neuroprotective effects. However, there is still much discussion regarding its efficacy, side effects, and practicality as a long-term treatment.
In many studies, marijuana has been shown to reduce IOP by interacting with the endocannabinoid system (ECS) present in the eye. Despite these promising effects, the reduction lasts only a few hours and may come with significant side effects. Our glaucoma specialists encourage patients to understand the science behind glaucoma treatment and the limitations of marijuana before considering any changes to their treatment plan.
Below, we break down the mechanisms involved, the research findings, and the concerns that arise when considering marijuana as a treatment option for glaucoma. Glaucoma and Intraocular Pressure Connection
Glaucoma is usually associated with elevated IOP, which results from a buildup of aqueous humor—a clear fluid in the eye produced by the ciliary body. This fluid naturally drains through structures like the trabecular meshwork and Schlemm’s canal. When too much fluid accumulates, the pressure inside the eye increases, damaging the optic nerve and leading to vision loss if untreated.
Lowering IOP is critical in managing glaucoma. Studies such as the Ocular Hypertension Treatment Study (OHTS) and the Early Manifest Glaucoma Trial (EMGT) have shown significant evidence that reducing IOP slows the progression of glaucoma. In these studies, even a 25% reduction in IOP resulted in up to a 50% decrease in the risk of worsening glaucoma. Given this relationship, any treatment able to reduce IOP merits consideration, which is why marijuana’s potential role has generated interest.

Marijuana's Impact on IOP
Studies have consistently shown that marijuana can decrease IOP by approximately 20–30% for a period of 3–4 hours after administration. However, this short duration demands repeated dosing throughout the day to maintain lowered pressure, which introduces significant challenges and concerns.

Benefits and Limitations: Research Findings
In controlled studies, marijuana has demonstrated a consistent ability to reduce intraocular pressure:
Comparison with Standard Treatments
Standard glaucoma therapies aim to maintain a consistent reduction in IOP across a full 24-hour cycle. These treatments include topical eye drops, laser therapy, and surgical interventions. Here’s how they compare with marijuana-based approaches:

and Challenges
Despite the documented IOP-lowering effect, several factors limit marijuana’s viability as a treatment for glaucoma:
